We’ll learn more about your practice, answer your questions, and walk you through the participation process from start to finish.

Prepare Your Application

To help us evaluate your participation request, we’ll ask you to provide documentation such as:

Practice or corporation W-9
Provider roster
Credentialing information for each provider, including:
- Active California medical license
- DEA registration
- Professional liability insurance
- Current curriculum vitae (CV)
- Board certifications, when applicable

Providers with an active CAQH profile may use it to streamline credential verification. If CAQH is unavailable, we’ll provide a credentialing application that can be completed for each provider.

Participation Review

Once we’ve received your application, our Provider Relations team will review your information, discuss participation opportunities, and guide you through the provider agreement. Our goal is to make every step transparent, straightforward, and responsive.

Credentialing & Network Approval

Our credentialing specialists will verify your professional qualifications and complete the enrollment process as efficiently as possible. We leverage industry-standard credentialing resources whenever available to reduce duplicate paperwork and accelerate approval.

Once your participation is finalized, you’ll be introduced to your dedicated Provider Relations representative and receive everything needed for a successful start—including onboarding resources, access to the provider portal, operational tools, and ongoing support.
